    1. Or somebody pulled the pin latch.

      A lot lizard worked her way through a TA lot just outside of Savannah. The truckers that turned down her services had their latches pulled. There were about a dozen trailers that dropped.

    1. The fifth wheel didn't lock on the kingpin.

      Whenever you hook up to a trailer, after you het the hoses and pigtail hooked up, you do a pull test.
      You lock the trailers brake and put the tractor in gear and see if the 5th wheel is locked (you can't move).
      If it's not, the tractor will pull up (a couple inches) and the trailer will stay put.

    2. In #1 the 5th wheel on the truck didn't latch, He turned and drove out from under the trailer.

    4. Stupidity.

      It wasn't latched in, so it slid off the plate, and the truck pulled out from underneath from right to left, leaving the right side unsupported, so it rolled over that way.

    5. KurtP.... The correct way to do a pull test is to do it BEFORE you hook up the hoses.. it would be a wasted step to get out, do everything then get back in and beside that, you always (when doing a correct hooking procedure) raise the landing gear when you hook everything up, doing it your way means if you tug too hard on the pull test, you may shoot out from under the trailer and it will fall to the ground, or onto the frame/off the 5th wheel....

  11. when I hook up, I look at 3 things in addition to the tug test, on the front of the 5th
    wheel there's a threaded end of a bolt with nut and washer that should be pulled tight, the release handle is pulled in and most important, lean under and make sure the jaws are locked around the pin, takes a whopping 30 seconds and I've never dropped one in 40 years
